The “Beverly Hills 90210” and “Riverdale” actor was surrounded by his family when he died at a Burbank, California, hospital on March 4, a week after suffering a massive stroke.
According to his death certificate, obtained by several news outlets, including USA Today and E! News, his service was handled by Taylor Funeral Home in Dickson County near Nashville.

The death certificate also listed the most common kind of stroke, an ischemic cerebrovascular accident, as the cause of death, USA Today reported. Perry's fiancée, Wendy Madison Bauer, was listed as his next of kin.
Perry was a part-time Tennessee resident since the mid-1990s, when he bought a farm in the small town of Vanleer, about 50 miles west of Nashville.
"The family appreciates the outpouring of support and prayers that have been extended to Luke from around the world, and respectfully request privacy in this time of great mourning," a Perry representative said in a statement to E! News after the actor's death.
