Crews responded to reports of a two-vehicle fatal crash around 11:55 p.m. on Old Troy Pike at Ingleside Avenue.
A 2015 Nissan Pathfinder traveled southbound on Old Troy Pike, traveled left of center and hit a 2017 Chevrolet Impala coming northbound head on.
The driver of the Pathfinder, identified as Rustam Suleymanov, 49, of Dayton was taken to Miami Valley Hospital with serious injuries, the OSHP said.
The Riverside Police Department, Riverside Fire Department and Sandy’s Towing assisted troopers at the scene.
