Additional staff members at the county Center for Adolescent Services, 333 Access Road, attemped to restrain the juveniles to prevent the escape, during which another staff member was assaulted.
Officers from multiple law enforcement agencies responded to assist due to an "officer needs assist" call activated. The area was secured, and all juveniles were detained and apprehended within the facility.
Once the incident was under control, all juveniles were accounted for and none had escaped. Two staff members suffered minor injuries and were treated on scene by the New Lebanon Fire Department.
The four inmates involved were taken to the main Juvenile Justice Center. They were charged with felony assault, vandalism and attempted escape. Additional charges may be filed, according to the sheriff's office.
The incident remains under investigation.

Two staff members were hurt during an escape attempt tonight involving four males at a juvenile detention facility.
"Four juvenile inmates attacked a staff member, tried to escape," said Sgt. Rick Moebius of the Montgomery County Sheriff's Office.
During the attempt, the youths cracked security glass on a door. Two staff members suffered minor injuries and were treated at the scene, but did not need to be taken to the hospital, Moebius said.
"Nobody actually did make it outside the facility," he said.
During the attack that happened around 7:30 p.m., a countywide '99' call was made for assistance because it was feared some youths may have made it out. The Montgomery County Sheriff's Office along with officers from New Lebanon, Trotwood and Perry Twp. responded to the facility, while officers from several other departments were called off while they were responding.
During the incident, the facility was placed on lockdown.
The four juveniles were taken to the main juvenile justice facility in downtown Dayton, Moebius said. They are facing felony charges for assault of a staff member, escape and vandalism.

A juvenile corrections officer trying to restrain several youths tonight led to a countywide '99' call for an officer in need of assistance.
The incident happened around 7:30 p.m. at the Montgomery County Juvenile Court's Center for Adolescent Services, 333 Access Road.
The disturbance brought officers from multiple jurisdictions to the juvenile detention facility, including the Montgomery County Sheriff's Office and police departments in New Lebanon, Trotwood, Harrison Twp. and Washington Twp., according to dispatchers. The incident involved several juveniles inside the facility.
The correctional and treatment facility, located in the village of New Lebanon, is equipped to house 40 boys and 10 girls adjudicated of felony offenses, according to the Montgomery County Juvenile Court website.
Corrections officers at the facility confirmed there was "an issue" but referred all questions to the director, who was not able to be reached for comment.
According to scanner reports, a medic was called to the scene, but it's not clear whether there were any injuries during the disturbance.
