Warren County restaurant inspections

The following are summaries of State of Ohio standard inspection reports on file, and accessible online, from the Warren County Combined Health District, from the week of Oct. 8-12. District information is available online at www.wcchd.com however inspection details are not online.

Acapulco Mexican Restaurant

916 Columbus Ave., Lebanon

Date of inspection: Oct. 12

Violations: Physical facilities: Maintenance and operation

Comments: A couple of fly strips, one in back of kitchen across from dry storage rack and in front of kitchen near tortilla storage rack, that have an accumulation of flies. Dead/trapped insects should be removed at a frequency to prevent their accumulation to prevent contamination.

Blue Ash Chili

4200 Aero Drive, Suite L, Mason

Date of inspection: Oct. 10

Violations: Food: Limitation of growth of organisms; Equipment: Protection of clean items

Comments: Pans nested on shelf does not allow for proper air drying. All sanitized items must be air dried to prevent bacterial growth and contamination of food.

Burger King

5558 Kings Mill Road, Mason

Date of inspection: Oct. 10

Violations: Food: Protection from contamination after receiving; Physical facilities: Maintenance and operation

Comments: Onions stored on the floor in small room next to office. All food must be stored 6 inches off the floor to prevent contamination. Base coving in poor repair by prep sink. Facility must be maintained.

Chan’s Asian Wok

8221 Arbor Square Drive, Mason

Date of inspection: Oct. 10

Violations: Food: Protection from contamination after receiving; Physical facilities: Maintenance and operation

Comments: Heavy dust build up on walls above to-go containers, open containers of peanut/cashews, food contact surfaces and in walk in refrigerator vent; to prevent cross contamination and for proper maintenance clean regularly.

Container without handle used as a scoop stored in direct contact to flour used for sweet-and-sour chicken; Possible cross contamination. Scoops shall have handles and not be in contact with food item. PIC discarded container.

Crème De La Crème (Deerfield)

5324 Natorp Blvd., Mason

Date of inspection: Oct. 10

Violations: Water, plumbing and waste: Plumbing system; Physical facilities: Maintenance and operation

Comments: Leak at underneath plumbing at wash side of three basin; For proper maintenance repair leak.

Heavy dust build up alongside oven hood, on top of reach in refrigerator and on wall behind refrigerator. For proper maintenance clean facility regularly.

Franklin Senior High School

750 E. Fourth St., Franklin

Date of inspection: Oct. 11

Violations: Equipment: Maintenance and operation; Physical facilities: Design, construction and installation

Comments: Kitchen very clean and well-organized. Good job. Door to walk-in freezer is in poor repair. Equipment must remain in good repair to ensure cleanability and functionality. Please repair door so that all parts are easily cleanable.

Light shield for fluorescent lights missing near hand sink. All light bulbs must have protective covering to prevent contamination should a bulb burst. Please replace cover for lights or install individual sheaths over bulbs.

Holly Hills Golf Club

4925 N. State Route 42, Waynesville

Date of inspection: Oct. 9

Violations: Equipment: Cleaning of equipment and utensils

Comments: Bottoms of reach-in Arctic Air cooler and Maxx Cold freezer have some food debris. All nonfood-contact surfaces of equipment shall be kept clean to prevent contamination and pest attraction.
